Application to be Excused from Jury Service form
The Application to be Excused from jury service is located on the back of your summons form. If you have lost or misplaced the form and wish to make an application to be excused please download this form and mail or fax the form (with appropriate supporting documents) to the Sheriff's Office listed on your summons. Failure to Attend form
If you have failed to attend for jury service you will be issued with a Failure to Attend form. This form gives you an opportuntity to explain in writing why you did not attend for jury service (Part A). Please include any supporting documents. You may also elect to have the matter heard by a magistrate at your local court (Part B). Mail or fax the form (with appropriate supporting documents) to the Sheriff's Office (Head Office - Parramatta) Locked Bag Box 5121 Parramatta NSW 2124 or by fax to: 02 868 89677 OR 86889676.
